We've created some fantastic Patient Information Packs for 5 Boroughs Partnership NHS Foundation Trust!
The Patient Information Packs provide a guide to services and support available to Service Users when they are discharged from 5 Boroughs Partnership NHS Foundation Trust in-patient care. They highlight important topics such as housing, money, relationships, care and treatment, which are needed when Service Users return to their communities.
The Packs consist of a folder and 10 insert sheets, which we designed and printed to be stepped so that the topic titles are immediately visible and accessible to all users. We selected appropriate imagery to accompany the information, and created icons to aid clarity and understanding.
In total, we produced 3,000 folders and 30,000 insert sheets!
